What personal data I collect and why I collect it
Clients who come to see me consent to providing personal data on a form provided by me as part of the process of making an appointment. This information is required by me so I am aware of your medical history allowing me to assess risks of and identify the appropriate treatment. Specifically you are asked to provide your name, dob, address, phone numbers, e-mail, occupation and medical history, current discomfort levels
If this history is provided electronically by email or contact form it will reside in my email box for up to 3 months then deleted. My PC is kept in a secure place, is protected by passwords and up to date with software to prevent unauthorised access.
A paper record of each treatment will be held with the form containing personal data and will be stored securely. At no time is any personal data or information about you shared by me to any company or 3rd party for any purpose. I use the contact details provided to communicate with clients about appointments, payment and treatment.
Contacting me via my website may result in cookies being stored on your computer. These cookies contain no personal data and is discarded when you close your browser.

How long we retain your data
Forms containing personal data and information provided electronically by email or contact form it will reside in my email box for up to 3 months then deleted. All clients can see, edit, or delete their paper record of personal information at any time. It is recommended by my insurer and professional association that the details of your treatment and appointments are held for 8 years after your final appointment.
What rights you have over your data
You have the right to access,read and check the data is correct. I am obliged to keep client records for administrative, legal, or security purposes for 8 years following the last appointment date. After this time your record will be destroyed.
